Prescription Errors
Many prescription errors cause harm only for a short period, but some result in serious injury, illness or even death. Prescription mistakes can be made by GPs or hospital doctors writing the wrong letter on a prescription or chart, or by pharmacies giving out the wrong drugs. Most dispensing errors occur at the pharmacy, but they do also occur in nursing and care homes.
Prescription errors can occur as follows:
- wrong medicine being prescribed
- wrong dosage of medicine being prescribed
- medication being prescribed for too long
- being prescribed medication to which allergic
- combining two or more medications which should never be taken together
- someone in your family that has died as a result ot taking a particular medication
